CalAIM Services for Justice‑Involved Individuals
Youth Recovery Connections provides specialized CalAIM (California Advancing and Innovating Medi‑Cal) services for justice‑involved individuals. These services support individuals during incarceration, reentry, and community reintegration by addressing physical health, behavioral health, and health‑related social needs that affect long‑term stability and success.
Our CalAIM programs reduce barriers to care, strengthen continuity of services, and promote successful transitions from incarceration to the community.



Eligibility
CalAIM justice‑focused services are available to Medi‑Cal members who are justice‑involved, including individuals who:

Are currently incarcerated and preparing for release

Have been recently released from jail or prison

Are on probation or parole
-
Have complex physical health, mental health, substance use, or social needs
Eligibility is determined by Medi‑Cal and the individual’s managed care plan.
Enhanced Care Management (ECM)

Enhanced Care Management (ECM) provides comprehensive, person‑centered care coordination for justice‑involved individuals with complex needs, particularly during periods of transition.
Services include:

Assignment of a dedicated care coordinator

Comprehensive health and psychosocial assessments

Individualized care planning

Coordination of physical health, mental health, and substance use disorder services

Assistance navigating Medi‑Cal, primary care, specialty care, and behavioral health systems

Coordination with probation, parole, reentry programs, and legal requirements

Ongoing advocacy and support to ensure continuity of care before and after release
ECM helps individuals remain connected to critical services during incarceration, release, and reintegration into the community.

Community Supports (In Lieu of Services)
Community Supports address health‑related social needs that directly influence successful reentry and long‑term wellness. Based on eligibility and availability, services may include:

Linkages to employment, education, and vocational resources
