Enhanced Care Management (ECM)

What Is ECM?

Enhanced Care Management (ECM) is a free service for eligible Medi-Cal members that provides one-on-one support from a dedicated care team. ECM helps people with complex health or social needs navigate services, coordinate care, and connect with resources that support their overall well-being.

Whether you're managing a health condition, experiencing a major life transition, or need help accessing community resources, our team works alongside you to create a plan that fits your needs and goals.

How ECM Can Help?

Your care team can help with:

  • One-on-one care coordination

  • Personalized care planning

  • Coordinating physical health, mental health, and substance use services

  • Connecting you with healthcare providers and community resources

  • Navigating Medi-Cal and other health and social service systems

  • Accessing housing, food, transportation, and other basic needs

  • Advocacy and ongoing support throughout your care

Who Qualifies?

ECM is available to eligible Medi-Cal members. You may qualify if you:

  • Have complex physical health, mental health, or substance use needs.

  • Are pregnant or recently gave birth.

  • Are involved in child welfare (foster care).

  • Are enrolled in California Children's Services (CCS) and have additional care needs.

  • Have frequent emergency room visits or hospitalizations.

  • Are transitioning from incarceration or are involved with the justice system.

  • Are at risk of long-term institutional care or need additional support living safely in the community.

Community Supports

Some people enrolled in ECM may also qualify for Community Supports. These services help address everyday needs that can affect health and recovery.

Depending on eligibility and availability, Community Supports may include:

  • Housing navigation and housing stability support

  • Reentry planning and post-release support

  • Connections to employment, education, and vocational resources

  • Assistance accessing food, transportation, and other basic needs

  • Connections to community services and peer support
